How to register with us

The doctors welcome new patients who live within our practice area.

Please either attend the surgery to complete a new patient registration form, or print off a copy from here and complete before you attend. PLEASE NOTE – We will require you to enter your NHS Number on this form. Your NHS number is displayed on any hospital letters or repeat medication slips. Alternatively you can obtain this number from your previous doctors surgery.

Our practice area covers the whole of Risca except Fernlea and postcodes beyond Oxford House. We also cover the Rogerstone area.
